The Agency for Healthcare Research and Quality (AHRQ) will host a March 31 Web conference on a toolkit to help hospitals improve their discharge process and reduce readmissions. The free event will feature Brian Jack, M.D., associate professor of family medicine at Boston University, who developed the Re-Engineered Hospital Discharge toolkit for AHRQ. The 75-minute Webcast is intended for hospital executives, clinicians and patient safety officers.
